Aims And Objectives

Services

Home Support/Domiciliary Care

We offer High quality support that is provided through a supported living setting. Care will be provided as defined by Schedule 1 of the Health & Social Care Act 2008 (Regulated Activities) Regulations 2014.

                                         We offer person centred support to:                                                              

  • Adults Over 65.
  • Adults with mental health concerns.
  • Adults with Dementia
  • People with a physical disability
  • Companionship

We are committed to providing a high-quality domiciliary care services which allows our services users to remain in their own homes with tailored support, promoting and maintaining their independence. This avoids admission into services like residential homes or nursing homes. We constantly strive to develop and innovate to ensure that our services users are receiving the best care. We provide responsive, safe, well led services and caring service. That delivers personal care to our service users. 

Supported Living-

We offer High quality support that is provided through a supported living setting. Care will be provided as defined by Schedule 1 of the Health & Social Care Act 2008 (Regulated Activities) Regulations 2014.

Our services will meet the requirements set out in the Real Tenancy Test and Registering the Right Support.

We offer person centred support to:

  • People between the ages of 18-65
  • Adults with mental health concerns.

The service will offer creative service levels in order to meet the needs of the population identified. Example: Low level support this will be suitable for people who live in the community but might need support in managing their bills and attending appointments.They can do personal care and some activities of daily living with minimal support. They may need floating support and use of assistive technology to seek help. Moderate level service: This is a shared living house were each service user hold their own tenancy , Staff are on site . This provision might be staff with sleep in staff or waking staff dependent on the specific needs of the service user. 

People who use this service might need 1 to 1 support during the week. Complex level: These levels will be able to carter for people with high levels of care needs. The service user might needs 1 to 1 or 2 to 1 support to remain safe.They may need 1 to 1 support in the community to remain safe when they are out. They may need a specialised environment that has reduced stimulus, anti-ligature risk assessment and reduced access to items that can cause harm. The service user will need a Positive Behaviour Support plan. We will increase independence by implementing the Personalisation agenda . The Personalisation agenda gives Service Users more choice, responsibility and control over the Services they receive. This should only be constrained by the realities of the limitations of resources available and the need to offer protection to vulnerable adults in a manner which is responsible but does not completely avoid risk. 

All Service users will require support to engage in reablement to move towards independent living, usually within 2 years. We will increase independence by using a coaching approach in order to build confidence in the community and ensure that services users are active members of the community. We will support service users to exercise their rights whilst supporting them to up keep their responsibilities. We will reduce the needs for more intensive support by using PBS support plan, in our experience in delivering support to service users the use of PBS has been shown to reduce the need for intense support .Recovery focused Supported is committed to ensuring that our service meets our customer’s needs. Throughout our service provision we ensure that we have a quality assurance process that ensures the care being delivered is in line with CQC’s standards of care.
